If you use the HylaFAX software it would be really useful to me if you would fill out the following survey form. This information is useful for finding out what modems are being used with the software and on what platforms. If you use HylaFAX with multiple different modem or on different systems, please submit one form for each modem or system (the forms are processed automatically).

Note that contact information is kept strictly confidential; it is solely for my use in supporting the HylaFAX software.
